OPS: Cuba reporta 163 casos de cólera, incluidos 12 turistas

La Habana, 27 ago.- Cuba informó de un nuevo brote de cólera en su territorio con 163 casos, doce de los cuales afectan a turistas, sin que se hayan reportado fallecidos, según una actualización epidemiológica de la Organización Panamericana de la Salud (OPS) publicada hoy en su web oficial.
El boletín de la OPS, con fecha del 23 de agosto, fue elaborado a partir de información suministrada al organismo por las autoridades epidemiológicas de la isla, donde el nuevo brote de la enfermedad tuvo lugar en las provincias de La Habana, Santiago de Cuba y Camagüey.
Hasta el momento ni las autoridades cubanas ni los medios locales han informado públicamente sobre la cifra de casos, aunque este martes un bloguero oficialista conocido como Yohandry divulgó en su página la información de la OPS.
De acuerdo con el boletín, entre los casos confirmados se encuentras 12 viajeros, de ellos dos alemanes, igual número de chilenos, españoles y venezolanos, un holandés y tres italianos, con rango de edad de entre 30 y 74 años.
"Todos ellos evolucionaron favorablemente y no se registraron defunciones", apuntó el reporte.
Las autoridades de la isla señalaron a la OPS que "durante el año en curso se investigaron casos sospechosos regularmente".
"Los casos registrados en La Habana estuvieron vinculados a dos centros de expendio de alimentos, en donde se comprobó la presencia de portadores asintomáticos entre los manipuladores de alimentos", añadió la nota.
Cuba informó además de que "en todos estos brotes se ha actuado oportunamente y se han realizado todas las acciones correspondientes para su control" y se "continúa implementando un plan de prevención y control de cólera".
El último informe público del Ministerio de Salud de la isla sobre cólera fue emitido en enero pasado, tras detectarse un brote en La Habana con 51 enfermos confirmados.
El país caribeño vivió cinco décadas sin casos de la enfermedad hasta que en agosto de 2012 el mal reapareció, afectando principalmente su zona este y con un saldo de tres muertos y 417 infectados, según datos oficiales.
La OPS reveló en este último boletín que Cuba registró asimismo otro brote con 47 casos confirmados tras el paso del huracán "Sandy" en octubre de 2012.
La última epidemia de cólera en Cuba ocurrió en 1882 y hasta el año pasado los últimos casos registrados databan de poco después de la revolución de 1959.

US Issues Advisory for Cuba Over Cholera Cases

The United States has issued an advisory for travelers to Cuba after several foreigners were sickened by cholera in recent weeks.
The message dated Tuesday urges visitors and U.S. citizens living on the island to avoid street food and under- or uncooked dishes such as ceviche.
The Pan American Health Organization says an Italian, two Venezuelans and two Chileans who traveled to Cuba contracted cholera in late July and early August.
Cuban authorities acknowledged a rare outbreak last summer after many years when the waterborne disease was not present on the island.
This January the government announced 51 new cases in the capital, but there has been nothing official since then.
Cholera can kill quickly through severe dehydration but is treatable if detected in time.

Los médicos tienen prohibido diagnosticar cólera, tienen que escribir en el certificado “enfermedad gastrointestinal”, menos puede el Ministerio de Salud Publica (MINSAP) decretar cuarentenas locales o informar a organismos internacionales de Salud, por que, oficialmente en Cuba no hay cólera. 
The doctors are prohibited from diagnosing cholera; they have to write "gastrointestinal illness" on the certificate, and still less can the Ministry of Public Health declare local quarantines or report to international health agencies, because officially there is no cholera in Cuba.

US SAFE FROM TWO NEW VIRUSES - FOR NOW

More than 50 travelers just back in the United States from China who had flu-like symptoms have been tested for the H7N9 bird flu virus, federal health officials say. So far, none has tested positive.

But the fact that they’re being tested at all shows just how worried the U.S. government is about this new strain of bird flu, which threatens at the same time as a still-mysterious coronavirus from the Middle East. The test kits had to be specially made up and distributed under an emergency provision.
“While no cases of H7N9 have been detected at this time in the U.S., 54 people with flu-like symptoms after travel to China have been tested. All have 54 tested negative for H7N9; while six tested positive for seasonal influenza A, and three tested positive for seasonal influenza B,” the Centers for Disease Control and Prevention says in its latest update on the virus.
Emergency operations centers are running 24/7, keeping an eye on both situations. While it's not unusual for the centers to be operating around the clock, it is rare to have two pandemic threats at once to plan for, says Edward Gabriel, who heads preparedness and response issues at the health and Human Services Department. 
"We want the latest and best information that we can get," Gabriel told NBC News. "We also need to look and see where it is moving to. To try to isolate its motion is a pretty significant thing."
If either virus turns into a form that spreads easily from person to person, a pandemic could follow within weeks. Both seem especially deadly in their current form: H7N9 seems to have about a 20 percent fatality rate, while the new coronavirus appears to have killed more than half its victims.
“In the case of the two latest threats — the H7N9 influenza virus and the new coronavirus — the number of infected people is small, and the infections are occurring thousands of miles away from the United States. Yet we should be seriously concerned about both,” Mike Osterholm, an infectious disease expert at the University of Minnesota, wrote in the New York Times on Friday.
“Our public health tools to fight these viruses are limited. We have no vaccines or effective drugs readily available to stop or treat the new coronavirus in the Middle East,” Osterholm adds. 

The H7N9 flu can spread silently, as people transmit influenza before they’re sick themselves. If the flu did mutate into a pandemic form, it would probably take at least six months to make enough vaccines to protect large numbers of people.
“It may take longer than it takes the virus to spread,” says Dr. John Treanor, a flu vaccine expert at the University of Rochester Medical Center. “The technology that we have today is such that the bulk of the pandemic disease may have already taken place before a vaccine is in place and can be used,” he added.
“The virus can spread very, very quickly. You are in a race against time.”
That happened in 2009, when the new strain of H1N1 swine flu broke out to cause the first pandemic of a new flu in 40 years. Companies raced to make vaccine but it was months before it was ready.
There are drugs to fight flu – a pill called Tamiflu and an inhaled powder called Relenza. Neither is a cure, however, and both need to be given very quickly to do much good at all.
Right now, H7N9 seems mostly confined to China and the spread has slowed. The World Health Organization reports 32 people have died out of 131 lab-confirmed cases.

Cuba: Confirman decenas de casos de cólera en Palmira, Cienfuegos

El periodista independiente AlejandroTur Valladares declaró al programa Cuba al día, que conduce Tomás Cardoso, que "este mes fueron diagnosticados 16 casos positivos de cólera en el municipio de Palmira (…) este dato fue informado por el Centro  Provincial de Higiene y Epidemiología. Estos 16 casos se encuentran en el Hospital Provincial Gustavo Aldereguia Lima, y sabemos de otros casos que están por confirmar aunque en principio han dado positivos (…) y se encuentran aislados en una escuela politécnica de este municipio".

Tur Valladares señaló que una funcionaria del Gobierno que él conoce, y que pidió el anonimato, le comunicó que "durante el fin de semana les notificaron que algo más de medio centenar de casos habían dado positivo en principio a partir de un test rápido que se les aplicó; esto es un examen de heces fecales, y que se espera la confirmación de otras pruebas realizadas que han sido enviadas al Instituto Pedro Kourí para confirmar un diagnóstico definitivo".

El periodista  dijo que "parece ser que este brote ha tenido lugar a partir de personas que viajaron de las provincias orientales al funeral de un ser querido en Palmira y portaban la enfermedad (…) ha sido el foco fundamental  a partir del cual se ha diseminado la enfermedad de una manera bien intensa".

"En el caso del municipio de Palmira no posee servicio de acueducto ni esta red de alcantarillado de aguas albañales no existe, el suministro del agua es a través de pozos y de letrinas o de fosas hacia donde van las heces fecales de la población y estas condiciones sin lugar a dudas han favorecido la extensión de la epidemia", indicó.

Deadly China bird flu outbreak raises fears of pandemic

Chinese authorities have closed some live bird markets in an attempt to stop the spread of a deadly strain of bird flu. A vendor, above, washed a chicken stall in a poultry market in Hefei, China, shortly before it was due to be closed Thursday.
BEIJING -- It began in late February when an 87-year-old man started coughing up phlegm. A high fever followed, he struggled to breathe and was dead just 13 days later. His death in Shanghai, China, was one of 11 fatalities out of 41 known cases to date of a new form of bird flu that experts warn may pose a "serious human health risk." On Saturday, China's center for disease control announced the first case in Beijing, and outside of eastern China. The seven-year-old girl, whose parents work in the live poultry trade, was stable in a hospital in the capital, media reports said. Around the world, scientists are now beginning to examine samples of the virus with a significant question in mind: Could this strain of the disease cause a global pandemic? This international network of scientists keeps constant watch for good reason. In 1918 and 1919, a flu pandemic killed between 20 million and 40 million people, more than the total death toll of World War I, more in a year than the Black Death of 1347 to 1351. More recently, an H1N1 swine flu pandemic was blamed for more than 284,500 human deaths worldwide between April 2009 and August 2010. So far, the signs are that this is a localized outbreak. The number of cases is low and the virus -- an H7N9 strain -- does not appear to be capable of jumping from one person to another. But each case represents a chance for the virus to mutate into one that is highly infectious in humans. And it is an unusual strain -- normally avian diseases make birds sick first, giving an early warning sign, but this one does not.
More than 1,000 dead ducks have been fished out of a river Sichuan, China. The discovery comes as the country deals with anger over the dumping of over 16,000 pigs elsewhere in China. NBCNews.com's Dara Brown reports.
Scientists have established it is from an "avian reservoir" but still don't know the precise source. Chinese officials have dismissed suggestions of a connection with the large number of dead pigs and other animals found recently in rivers. Many in China are understandably worried, with some deciding to avoid eating chicken, even though it poses no threat if properly cooked. KFC’s parent company Yum reported on Wednesday that sales in its Chinese restaurants had dropped by 13 percent in March, saying “publicity associated with avian flu in China has had a significant, negative impact.” Even Jiangsu Zoo, just north of Shanghai, reportedly stopped feeding chicken to animals such as lions and tigers and started giving them a traditional medicinal herb called ban lan gen. Xie Li, an accountant in Shanghai, admitted she was “kind of nervous.” “Now, we only eat vegetables," she said. "My daughter's school is measuring students' temperatures. We were told that we should eat less eggs or not touch eggs because they might have some excrement from chickens." But others in the city of 23 million people were more sanguine.
A farm in China has admitted to dumping more than 6,000 pigs corpses into Shanghai's Huangpu River, according to China's official Xinhua news agency. NBCNews.com's Alex Witt reports.
Yan Zhanlin, a 40-year-old businessman, said he was “not scared, because there are not many cases, and the number of deaths is not high” and the virus had not yet spread between people. “Today, I went to a train station, and I only saw few people wearing masks,” he said. But even he said he had stopped eating “poultry, pork and other meat.” Tang, a company manager in his late 20s, who declined to give his full name, was also relatively unconcerned. “I do not fear [the virus] at all. It is just a kind of flu, and will pass quickly,” he said. Avoiding poultry was “not too bad, because it forces us to eat vegetables and fish, which are nutritious,” he added. 'Watching very carefully'Perhaps in a sign of the country's nervousness, People's Liberation Army Colonel Dai Xu claimed the U.S. was behind the outbreak, saying the U.S. had used "bio-psychological weapons" to cause the deadly 2003 Sars outbreak and the current flu one, The South China Morning Post reported. Such allegations aside, this apparently local problem is being treated seriously on a global scale. Samples of the virus – or non-infectious nucleic acid from it — are being sent to scientists in up to 140 national influenza centers recognized by the World Health Organization, including the Centers for Disease Control and Prevention. Work has already started in the U.S. to make a vaccine against the new strain -- just in case. Scientist John McCauley, of the U.K.’s National Institute for Medical Research, received his consignment on Thursday. “We’re watching very carefully the events there [in China] because we are aware although there’s no human-to-human transmission, these are unusual infections people have been getting from an avian reservoir,” he said. “China will need to identify the source and hopefully be able to control the cross-species transmission,” he said. “We’re watching very carefully to see how it does.”
The outbreak of a new strain of bird flu has now infected at least 18 people, and killed six in China. NBC's Robert Bazell reports.
“In the meantime, the national influenza centers around the world are developing their ability to detect this newly emerging virus” and also working on vaccines, McCauley said. Experts needed to find out how vaccines would perform “in case this virus becomes pandemic,” he said. Coincidentally, John Oxford, a professor of virology and an expert on the 1918 flu pandemic, was in Shanghai about eight weeks ago -- roughly the same time that the elderly man first fell ill – for a meeting about hygiene, important in the fight against viruses such as flu. He said the situation in China was “getting a little more worrying.” “I don’t like the sound of it. Every day I open up the reports and find out someone else has died,” he said. “I just don’t like to see the figures going up day after day.” “So far there’s no human-to-human transmission. What’s tomorrow going to bring, what’s the next day going to bring? You don’t know and I don’t know,” he added. But Oxford, of the U.K's Queen Mary, University of London, stressed there was “no need for anyone to start flapping at the moment.” “I don’t think we should start thinking of 1918 scenarios, definitely not,” he said.
A group of Chinese scientists, writing in the New England Journal of Medicine, also warned that the “pandemic potential of these novel avian-origin viruses should not be underestimated.” “Severe avian influenza A (H7N9) infections, characterized by high fever and severe respiratory symptoms, may pose a serious human health risk,” it added. “We are concerned by the sudden emergence of these infections and the potential threat to the human population.” However – mirroring the split on the streets of Shanghai – other experts were less worried. Adolfo Garcia-Sastre, a microbiology professor at Mount Sinai School of Medicine in New York and principal investigator for the Center for Research on Influenza Pathogenesis, said while it was “too early to be able to conclude anything …  the probabilities are very low” that a global pandemic is looming. He was comforted by the lack of a surge in the numbers of people with the disease. “It’s not that it’s increasing by ten times per week, I think right now the number of cases is what you would have expected from the original numbers,” he said. “Right now there are no major indications to become highly alarmed.” Ian Johnston reported from London.

Cuba: Autoridades confirman cólera en Holguín; niegan fallecidos

Las autoridades cubanas confirmaron la existencia del cólera en la ciudad de Holguín, aunque negaron que se hayan producido muertes.
Una nota oficial de la Dirección de Salud Pública en la provincia holguinera indicó que 22 personas -entre ellas 9 niños- fueron diagnosticadas con cólera, contraído a través de un "portador asintomático de la enfermedad que manipuló los alimentos consumidos" en una fiesta de cumpleaños.
Actualmente se mantienen hospitalizadas 10 personas, indicó el comunicado, al que tuvo acceso CaféFuerte.
La información oficial corrobora la versión difundida a comienzos de febrero, aunque las autoridades negaron que hubiera fallecidos a causa del cólera. El 1 de febrero, CaféFuerte reportó por primera vez el brote de la enfermedad en Holguín e informó de la muerte de dos mujeres.
CaféFuerte sostiene que se produjeron ambos fallecimientos.
Desde que a mediados de junio 2012 se desató el primer brote de cólera en Cuba desde el siglo XIX  las autoridades sólo han reportado tres fallecidos, a pesar de que fuentes independientes ubican la cifra en alrededor de una decena de muertes. A mediados de enero, la BBC identificó a Osvaldo Pino Rodríguez, de 46 años, como uno de los fallecidos a causa del cólera en La Habana, pero su deceso nunca fue reconocido por el Ministerio de Salud Pública ni reportado en los medios oficiales cubanos.
A mediados de la pasada semana establecimientos e instituciones públicas de la ciudad de Holguín comenzaron a colocar recipientes con agua clorada en la entrada, una medida de prevención que se ha convertido en familiar en provincias como Granma, Santiago de Cuba y Guantánamo.
Este es el segundo brote de cólera en Holguín desde noviembre.
A continuación reproducimos el comunicado emitido por autoridades sanitarias de Holguín:
"La Dirección Provincial de Salud de Holguín informa que a partir del domingo 27 de Enero el Sistema de Vigilancia Clínico Epidemiológico detectó un incremento de las Enfermedades Diarreicas Agudas en el Municipio Holguín, Área de Salud Máximo Gómez. Se determinó que correspondía a un brote de Enfermedad Transmitida por Alimentos (ETA), ya que todos los casos tenían como elemento común haber participado en un cumpleaños el viernes 25 (48 horas antes).
Como parte de la vigilancia  se realizaron los estudios correspondientes, ingresaron 43 pacientes, 16 menores de 18 años. Se confirmó la presencia del Vibrión Colérico en 22 casos (9 niños y 13 adultos). Según las evaluaciones epidemiológicas efectuadas la transmisión se generó por portador asintomático de la enfermedad que manipuló los alimentos consumidos. La evolución clínica ha sido favorable, se mantienen ingresados 10 pacientes, no reportándose fallecidos por esta causa.

Como resultado de las medidas adoptadas el brote se encuentra controlado. Ello ha sido posible por la vigilancia permanente y la capacidad de respuesta del Sistema de Salud, con la activa participación de los organismos y organizaciones de masas dirigidos por las autoridades del Gobierno y el Partido.

Se reitera la necesidad de intensificar las medidas higiénicas, especialmente las relacionadas con el lavado de las manos, la ingestión de agua clorada, la manipulación y conservación adecuada de los alimentos y no consumirlos cuando se desconoce
su procedencia, aspectos estos imprescindibles en el cuidado de la salud.

CDC Issues Cholera Travel Notice for Cuba

On January 6, 2013, The Cuban Ministry of Health (MoH) confirmed an outbreak of cholera in Havana, the country’s capital. A total of 51 laboratory-confirmed cases of cholera have been reported in Havana.

In July 2012, the Cuban MoH confirmed the country’s first cholera outbreak in more than a century. That outbreak was in the city of Manzanillo, in eastern Granma province, and was declared over in late August. Cuba’s cumulative number of confirmed cholera cases since July 2012 is now more than 500.

Cuba and Cholera: Good Hygiene and Good Government Can Save Lives

Almost five months ago, the Cuban government announced the end of a cholera outbreak in eastern Cuba. At the time, Cuba’s Public Health Ministry blamed the three deaths and 417 cases on overflowing toilets, heavy rains and contaminated wells. According to the government, thanks to its prompt reaction and the quality of the country’s public health system, the crisis ended quickly.

Just recently, on January 15, the Cuban government announced that there were “only” 51 new cases—this time in Havana.

International and independent media on the island had already reported the outbreak, with independent media doing so months ago. On January 13, the BBC reported a “cholera fear in Cuba as officials keep silent.” Likely prompted by the international attention, the next day the government published an Information Notice to the Population saying it had detected “an increase in serious diarrhea…with symptoms that lead to the suspicion of cholera.”

Lest anyone become concerned, however, the government stated in the notice that it had everything under control and an “anti-cholera plan…was immediately activated” in which the government has “all the means and resources needed.” The Information Notice offered no additional details other than to say that “due to the measures already taken, the outbreak is on its way to extinction.”

International news reports, however, belied the Cuban government’s claims. The January 13 BBC story reported that one known death had occurred “in one of the poorer and more overcrowded districts of Cuba’s capital” and that “there are increasing signs” of more cases. “Suspected cases are being sent to the Tropical Medicine Institute…All our wards are dealing with this issue—they are almost full,” an Institute employee told the BBC.

As a result of their own independent assessments, both the British Embassy and the American mission in Havana issued travel advisories earlier this week, and other embassies told the Associated Press that they were considering doing the same. The diplomats told AP they “have been concerned that the government is not sharing information with them in a timely manner”

An AP report that appeared in The Miami Herald on January 15 explained that “Cholera is a waterborne disease caused by a bacteria found in tainted water or food. It can kill within hours” and said that “it was unclear why a new outbreak was being seen in Havana. Rains, which can help spread the disease, are common in January, but the weather has been unusually dry this year.”

Blame Game

To blame the weather (as well as the United States) for all of Cuba’s misfortunes is a common practice of the Cuban authorities. Years ago, Havana claimed the United States was responsible for starting epidemics, and even a tobacco plague, years ago, but when Washington demanded proof they had nothing to show. What the repeated outbreaks and the Cuban government’s efforts to conceal them show is that the much-heralded Cuban public health system is a great sham. The regime makes a great show of sending its medical personnel abroad to countries like Venezuela and Bolivia, while closing a number of clinics and hospitals at home and sending doctors who disagree to prison.

Moreover, the persistence of cholera is also laying bear the sorry state of Cuban infrastructure, much of which has not been updated since before the revolution. Unfortunately, the international media has failed to make the connection between the disastrous conditions of Havana’s—and other cities—water and sewage systems and the cholera outbreak, as they would if they were reporting in Haiti or another country. But the truth is that thousands of Havaneros do not have access to running water, a service long since cut off to the homes of many ordinary citizens. Instead, they must patiently wait in line for the government trucks that are supposed to regularly distribute water (but rarely do) for drinking, washing and cleaning.

Today, the government insists that the tourists are fine because they all can have bottle water. But what does that mean for the country’s own citizens?

With a population more than twice what it had in 1959, Havana still depends on the same outdated water systems: its aqueduct, pumps and pipes have served the city since before the Castros came to power. Nationally, more than half of the water supply is lost due to leaks and improper maintenance, according to Cuba’s Institute of Hydraulic Resources. The government’s response? It recommends that Cubans dig their own wells for potable water.

There is also the problem of Cuba’s outdated and short-changed sewer system. The lack of maintenance of the country’s creaky system for safely disposing of human waste is made worse by pipes that dump untreated sewage directly into streams and on to shores close to populated areas. Shoreline pollution is exacerbated by official corruption and the pilfering of pipes, which results in the pollution spreading farther on land.

Months ago, dissident journalists reported several deaths attributed to cholera in neighborhoods south of Havana. The epidemic began hundreds of miles away in the city of Manzanillo, but there have been cases in other provinces.

Cuban authorities tried to blame the self-employed for the cholera and responded by prohibiting the sale of lemonade and other fruit drinks. There have been reports of closing of schools. Visitors are not allowed to visit cholera patients at various hospitals. And there are shortages of soap and cleaning products, which often can only be purchased at hard currency stores. But the majority of the population has no dollars.

Tragically, in the absence of real information and honesty on the part of the government, the only way Cubans learn about the situation and how to protect themselves is from independent media and civil society. Last summer, during the outbreak in the eastern provinces, Raúl Castro complained that dissidents and the foreign media had exaggerated the crisis. But without the independent journalists, Cubans working in the government hospitals who talk to them and foreign correspondents, the government would have been successful in covering up the epidemic.

Last Sunday, Cubans attending a number of churches were told to pay special attention to water and hygiene. But this is easier said than done: typically, three generations are forced to share a single small house or apartment. In this case, as in others, Cubans learn about developments on the island by listening to foreign broadcasts from South Florida, Dominican Republic and Colombia.

The Castro brothers, like other totalitarian rulers, rewrite history on a regular basis. But while the regime boasts about Cuba being a medical superpower Cubans continue to die of a nineteenth-century disease. The last recorded cholera epidemic in Havana took place in 1883.

Camilo, my 7-year old grandson in Cuba, has never been shy about asking for presents – especially when he knows I’m heading to Havana from that big shopping mall 90 miles away. His usual list includes a massive bag of M&M peanut candy, additions to what’s become a pretty pricey collection of Schleich resin animals, and goofy gags second-grade boys find funny, such as hand buzzers or that classic snake-in-a-can. When Camilo got on the phone with me last weekend, he only rattled off one item.
“Aba,” (that’s what he calls me–short for "abuela", which is "grandma" in Spanish), “bring me soap.”
“Soap? You want soap?” I repeated, convinced I must have heard him wrong.
“Si”, he insisted. “Jabon!”
Now he has me worried that I need to make an emergency supply-run for detergent, shampoo, dishwashing soap and other basics. The last time soap was in short supply in Cuba was in the 1990s but, if this kid is asking for soap, the situation must be dire. He’s about as germaphobe as your average stray puppy. Like a lot of little boys, he needs to be reminded that taking a shower means actually standing under the water.
Camilo, however, didn’t want just any soap. He was looking for what he calls “the soap that melts.” He wanted me to bring him an alcohol-based instant hand sanitizer.
Then he made it clear why. “Aba, there’s cholera here,” he said.
As it turns out, Camilo had spilled the beans a full 72 hours before the Cuban Health Ministry issued a formal communiqué on what had been rumored since the start of the year -- cholera had surfaced in the city of Havana, home to 2.2 million people.
The announcement explained that 51 new cases of cholera had been diagnosed in the Cuban capital along with a spike in the number of people suffering from "diarrheal diseases." The ministry made no mention of any fatalities. The public was being advised to be more careful with personal hygiene, boil all drinking water or use purification drops and thoroughly wash all fruits and vegetables, but to stay assured that Cuba’s massive public health machine was implementing preventive measures meant to “contain” and “eradicate” the disease.
According to the Centers for Disease Control and Prevention website, cholera is a bacterial infection in the intestine that can range from mild to severe. In the latter case, an infected person will experience “dehydration and shock" that, if left untreated, can lead to death "within hours.” The CDC estimates that every year there are up to 5 million cases and more than 100,000 deaths from cholera worldwide.
In most cases, the disease takes about a week to run its course, and during that time, warns the CDC, cholera is highly contagious. Spread hand to mouth, the bacteria is usually found in water or food sources contaminated by an infected person’s feces.
ContaminationA single food vendor at a baseball game appears to be the cause of the Havana outbreak. In early January, apparently, contaminated sandwiches or soda were sold during a packed game in the city’s main sports arena, the Latin American Stadium, located in a neighborhood called Cerro.
"That's why people from different parts of the city tested positive for cholera at the same time,” said a medical source, not authorized to speak on the record but who claims to have first-hand knowledge of the findings from the epidemiological task force assigned to trace the origins of the outbreak.

At Wednesday's nighttime game between Havana's beloved Industriales and last year's national champs, Los Tigres de Ciego de Avila, hawkers should have been making a killing on what had been one of the season's most sought-after tickets. Instead, 80 percent of the seats remained empty. Those die-hard fans who did show up were not allowed into the stadium until they sterilized their shoes and hands. Benches were wiped down with a disinfectant, and the floors hosed down with the same 0.5 percent bleach solution. And there was nothing to munch on during the three-and-a-half-hour game. All food stands have been temporarily shut down.
The same goes for many mom-and-pop cafeterias across the capital. "Last week, officials from Health and Epidemiology inspected our place and then they closed us down," said one owner of a pizza parlor not too far from where the outbreak started. "They said it's to stop the spread of cholera but no one’s saying how long we have to stay closed." His only consolation is that this month he doesn’t have to pay taxes or his monthly licensing fee.
Upset about his loss of income, he is also irked by the fact that some state-run food establishments passed the inspections, so they are being allowed to stay open. Many though are only authorized to sell bottled water, canned drinks and commercially packaged food.

He stands by the one door opened at the station and his job is to pour a diluted bleach solution over the hands of every commuter, and make sure everyone steps on a cloth soaked with the solution to clean the bottom of their shoes. No one gets on a bus or leaves the building without Nieto’s OK.
Now, picture this procedure happening at every school from kindergarten to college, every public building, factory, lunch room, hospital, health clinic, department store, train depot and movie theater.
Chlorine a "necessary inconvenience"
Over the summer, two people who live in the Havana neighborhood of Fontanar thought they had the flu but tested positive for cholera. It was believed that they were exposed on the bus ride from eastern Cuba, an area of the country that had an outbreak earlier last year. In late August, Cuba revealed that cholera had killed three people and infected 417 in Granma province, some 450 miles east of Havana.

Cuba’s cholera treatment protocol has doctors knocking on doors and testing anyone with possible cholera symptoms. A positive test means an automatic trip to one of the city hospitals for a more comprehensive test. While most suspected cases go to Havana’s Tropical Medicine Institute, known by its initials IPK, a pediatric hospital and a maternity hospital have also been designated to admit cholera cases. In addition, the protocol mandated that all of Havana’s 85 neighborhood health clinics set aside a room with ventilation and a closed door as a place to quarantine suspected cholera cases until an ambulance arrives to transport the patient to the hospital.
Once hospitalized, a comprehensive history is taken that focuses on identifying all the people the patient has come in contact with over the past weeks. Health workers are dispatched to locate those persons to test them for cholera and administer a free prophylactic dose of doxycycline.
Although none of the guidelines cited by the CDC recommend using antibiotics for cholera prevention, the Cuban Health Ministry believes otherwise. Hundreds of thousands of Doxycycline tablets, apparently readied in warehouses for just such an emergency, were distributed to hospitals and health clinics one morning earlier this week—another sign that Cuba is well-prepared to tackle this outbreak.
Are people complaining? You bet. They hate the chlorine smell. They say the solution stings but many would agree with Angela Linares, a nurse raising a 13-year old daughter alone, who said: “It’s a necessary inconvenience.”
“No one wants cholera, especially since we know so little about this disease,” she said.
Linares was right. Until last year, the last reported cholera outbreak in Cuba was recorded almost a century ago.
Upon learning this fact, I became even more baffled that my 7-year-old grandson mentioned cholera days before the government admitted the outbreak.
As it turns out, his primary school had been put on alert early last week, and the kids learned about the intestinal bug and prevention at a school assembly. Community physicians were dispatched to all of the city’s 650 schools to not only give a crash course on cholera but hand out soap to every classroom.
Still, it wasn’t until after the Health Ministry’s warning that Cuban state media began running public service announcements -- considerably behind the curve of Havana's second graders.

TORONTO - Authorities in Cuba are acknowledging a spate of new cholera cases in the capital, but the outbreak doesn't seem to be keeping Canadians away.

Travel agents say January is always a popular time for bookings to Cuba and they say business hasn't tailed off in the least.

Some agents say word of 51 new cholera cases in the Havana area has only sparked a handful inquiries from people who have no plans to cancel their trips.

The Public Health Agency of Canada has issued a low-key advisory about the outbreak, but says most travellers are at low risk of contracting the intestinal illness.

Cuba's health ministry says no one has died from the latest batch of cases, which began surfacing earlier this month.

It says the country has taken preventive measures to quash the disease for good.

New Cholera Outbreak in Cuba Calls Countries to Restrict Travel

Fifty-one new cholera cases reported in Cuba’s capital are drawing an increasing concern about the spread of the illness throughout the country.
Cuba's Public Health Ministry acknowledged the situation in Havana on Tuesday as disappointment rises in the diplomatic community over the government's lack of transparency.
While the outbreak, which began Jan.6, has not caused any deaths, the government has stressed the preventive measures already taken have put the disease "on the way to extinction."
The government has not responded to repeated requests for comment, nor has it made any experts available to talk about the cholera situation.
According to the World Health Organization, almost 80 percent of cholera cases can be treated with oral rehydration salts.
Today there are two variants of oral cholera vaccines available, although neither guarantees 100 percent immunity from the disease. While only offering “moderate protection,” the use of the vaccine can help develop “herd immunity,” said WHO officials.
Cholera is a water-borne disease caused by a bacteria found in tainted water or food. It can kill within weeks through dehydration, but is treatable if caught in time. Cholera is unusual in Cuba but recent outbreaks in nearby Haiti have killed more than 7,200 people.
It was unclear why a new outbreak was being seen in Havana. Rains, which can help spread the disease, are common in January, but the weather has been unusually dry this year.
As of Tuesday, the British Embassy in Havana had issued a travel advisory urging its citizens to take "sensible precautions" and seek immediate medical attention for diarrhea.
Several other European diplomats told AP they are also considering issuing similar advisories, and have been concerned that the government is not sharing information with them in a timely manner. They spoke on condition of anonymity because they were not authorized to speak publicly.
As far as the U.S., American diplomats on the island issued a travel warning Monday urging American citizens to follow local health recommendations. About 400,000 Cuban-Americans visit the island every year, and about 100,000 others issued visas.
The nationalized health care system in Cuba is considered to be one of the most advanced in Latin America. Because of this, Cuba has become a population destination for health care tourism.
So far, no tourists have come down with the illness.
Tourism in general is the top sector in Cuba's flagging Communist economy with 2.8 million visitors a year and about $2.5 billion in annual revenue. A major cholera outbreak could hurt tourism. 
While Cuba's state-run media had been largely silent about cholera before Tuesday, there has been an intensified campaign against water-borne diarrhetic illnesses. Several health centers in the capital require visitors to sanitize their shoes by stepping in chlorine when they enter, and state schools have been stressing hand-washing and other hygiene measures.
"I know that they are taking the necessary measures," said Beatriz Guerra, a 26-year-old mother of two who lives in Miramarshe. "One just needs to be very cautious."

Reconoce Cuba 51 casos de colera en La Habana

NOTA INFORMATIVA A LA POBLACIÓN 
El Ministerio de Salud Pública informa que a partir del domingo 6 de enero el sistema de vigilancia clínico epidemiológica detectó un incremento de las enfermedades diarreicas agudas en el municipio Cerro y posteriormente en otros municipios de la capital. Un grupo de estos pacientes presentaban síntomas y signos que orientaban etiológicamente a la sospecha de Cólera, lo que determinó la activación de las acciones previstas en el plan anticolérico, para cuyo cumplimiento disponemos de todos los medios y recursos necesarios.
El análisis microbiológico realizado por el Instituto de Medicina Tropical "Pedro Kouri", determinó que el agente causal es el Vibrio Cholerae O1 Tor enterotoxigénico serotipo Ogawa, estando confirmados hasta la fecha 51 casos.
Según las evaluaciones epidemiológicas efectuadas la trasmisión se generó por un expendedor de alimentos, portador asintomático de la enfermedad, adquirida durante los brotes informados con anterioridad en otras regiones del país.
Como resultado de las medidas adoptadas la trasmisión se encuentra en fase de extinción. Ello ha sido posible, en primer lugar, por la vigilancia permanente y la capacidad de respuesta de nuestro sistema de salud.
Se reitera la necesidad de intensificar las medidas higiénicas, especialmente las relacionadas con el lavado de las manos, la ingestión de agua clorada, la limpieza y cocción adecuada de los alimentos, aspectos estos imprescindibles en el cuidado de la salud.

Cuban barber died of cholera - Fear in Cuba as officials keep silent

Uvaldo Pino was a neighbourhood barber in Cerro, one of the poorer and more overcrowded districts of Cuba's capital, Havana.
In late December, the 46-year-old fell sick with vomiting and diarrhoea and died in hospital on 6 January.
The barber's family say he had two separate tests and both came back positive - for cholera.
"We don't know how he was infected," his sister, Yanisey Pino, told the BBC at the family's home, a few blocks from the capital's Revolution Square.
"He was treated, he had all the medicine, but his organs didn't respond. It was too late."
Yanisey added that her brother was a heavy drinker and had checked himself out of hospital the first time he was admitted.

A week after Uvaldo's death, Cuba's health ministry has not yet made any public pronouncement. But there are increasing signs that the barber's case is not an isolated one.
'Dozens' of admissions
Doctors are now making door-to-door enquiries in Havana and anyone displaying possible cholera symptoms is being tested. Suspected cases are being sent to the Tropical Medicine Institute, the IPK.
"All our wards are dealing with this issue - they are almost full," an IPK employee told the BBC by telephone, before saying she was not authorised to comment further.
Another staff member, contacted later and also not authorised to speak to the media, said the IPK did not have any confirmed cases of cholera at this point.
But Yanisey Pino says her brother was diagnosed with cholera both by his local hospital and the IPK.
The day Uvaldo died, health workers visited the family where they live - in several cramped houses around a small yard. Relatives and neighbours were issued antibiotics as a precaution.
The area has been disinfected and water samples were taken for testing. Meanwhile, nearby bars and cafeterias have been closed or instructed not to sell food or drink that is not pre-packed.
Elsewhere in the neighbourhood, there are similar scenes.
One resident, Yudermis, fell sick just before the New Year, along with four other relatives including her seven-year-old son. The family assumed they had food poisoning but Yudermis says her cousin then tested positive for cholera at their local clinic.
"The health workers then came here asking questions, like if we had diarrhoea," she explains inside their rundown family home as her son, now fully recovered, plays nearby.
"They sent us all to hospital by ambulance and the tests came back positive.
"There were a lot of people at the IPK," Yudermis adds, describing dozens of admissions while she was being treated, and not all from her own district of Cerro.
"I was in a bad way. It was frightening. But we're fine now."
Before she fell sick, Yudermis had never even heard of cholera, which is rare in Cuba.
Cold grills
The World Health Organisation (WHO) describes cholera as "extremely virulent". Carried by contaminated water or food, it causes severe dehydration through diarrhoea and can prove fatal if untreated.
Until last summer, there had been no significant outbreak on the island since well before the revolution.
But in July the health ministry confirmed that three people had died of cholera in the east of the country. A contaminated well was identified as the source.
In Havana, Cuba's bustling and crowded capital and a key tourist centre, strict measures are in place to contain the latest suspected outbreak.
"We can't sell anything that's not in sealed bottles until further notice and all food sales have been suspended," explains Tony, at the Cerro Moderno cafe, a short walk from the home of Yudermis. Its fridge is now empty and the grills cold.
Local doctors confirmed this is standard procedure for several blocks around every location where someone tests positive for cholera.
"If they take all the right measures, we'll be fine," Tony shrugs, adding that everyone has been given antibiotics as a precaution.

"I don't know what cholera is and I don't want to find out. People here are using chlorine and boiling their water. You have to take care."
Rumour mill
Pharmacies across the city are now selling water purification drops, rationed to two small bottles per person.
But in the tourist heart of Old Havana, cafes and restaurants remain open and the streets are still full of mobile food and drink vendors.
Most say they have heard rumours of a cholera outbreak in Cerro and are taking extra precautions, but none have received any official instructions.
The WHO stresses "public communication" as a key tool in controlling any cholera outbreak.
In Havana, that task has so far been left to local doctors who are very connected to their communities.
But as rumours fill the information void, concern on the streets is growing.
"I'm racking my brains trying to understand why there's nothing on TV about this," says Yanisey Pino, echoing many peoples' comments.
"Why don't they say something? Inform people, like in other countries, so they're not afraid and can protect themselves! But there's no information at all."

Havana put on alert over cholera outbreak

The Provincial Health Office this Monday confirms that government authorities have declared a high alert for sanitary conditions in three of the 19 municipalities in the Cuban capital due to a cholera outbreak (IPS).

Until now, the majority of the cases have been reported in the municipalities of Cerro, 10 de Octubre, and Regla, although cases found in six other municipalities have been classified as isolated.

Dr. Hernan Madera, an epidemiology specialist who heads a multidisciplinary group created in Cerro, reports that those who have been infected are immediately transferred to the Pedro Kouri Tropical Medicine Institute (IPK) for treatment.

It is reported that almost fifty individuals infected with cholera have been admitted into the IPK.

Se propaga la tuberculosis en Quivicán [Cuba]

LA HABANA, Cuba, 10 de enero de 2013, www.cubanet.org.- Innumerables avisos alertan y sugieren a los pobladores acudir inmediatamente a policlínicos comunitarios, tan pronto presenten síntomas de esputos sanguinolentos con fiebres y sudoraciones nocturnas. Los carteles han sido pegados en lugares públicos del poblado de Quivicán, localizado en la provincia de Mayabeque, al sureste de La Habana.
La mayoría de los impresos―según las fuentes—se encuentran expuestos en murales de escuelas primarias y secundarias de la localidad, puesto que los niños y los adultos mayores (tercera edad) constituyen los grupos de mayores riesgos.
Las fuentes consultadas, que solicitaron el anonimato, no pudieron obtener estadísticas que enumeren la cantidad de enfermos en correspondencia con la demografía del municipio. Es de suponer que la cifra es elevada y preocupante, ya que el régimen se esfuerza en oscurecer aquellas informaciones que pongan en tela de juicio la eficiencia de su sistema nacional de salud.
A finales del año 2011, el general / presidente, Raúl Castro, declaró que el “secretismo en Cuba había acabado”, pero las informaciones sobre las cifras de infestados y fallecidos a causa del dengue hemorrágico, la tuberculosis y el cólera en Cuba, está fuera del alcance de los medios de prensa.
El pasado año, la directora general de Organización Mundial de la Salud (OMS), Dra. Margaret Chan, visitó a Cuba y quedó “asombrada y complacida” con los avances de la Salud en Cuba. Lo dijo desde el aeropuerto y sin haber visitado un solo hospital o policlínico comunitario. Sin embargo, esta organización que presta asesoría técnica y recopila las estadísticas sobre el control de epidemias y las cifras de infestados y muertos, en sus informes anuales ofrece números que no coinciden con la realidad que estamos viviendo.
En un boom propagandístico de los años 80, el dictador Fidel Castro anunció al mundo que “Cuba sería una potencia médica”. En la actualidad, las infraestructuras de los hospitales están arruinadas, son preocupantes las carencias de fármacos, equipos y enseres. Entretanto, los profesionales y técnicos de la salud han manifestado su descontento por la falta de incentivos, lo cual revela  que el sistema de salud cubano cayó en un bache definitivo.
